Hospice Patients Acquired

“We were able to learn that some of our former patients had declined since coming off of our services and really would benefit from our hospice line of care. We were able to follow up with the patients and their physicians and coordinate appropriate quality care. I am so grateful for Cortex as it has helped us bring several patients back on service, as well as helped us catch things up front so we didn’t end up having dissatisfied patients.”

Former Patient Welcomed Back

“I followed up on a rehospitalization alert I received from Cortex and noticed one of our patients had been readmitted to a local hospital for a left total knee. We had seen him previously for a right total knee. I notified our team and they called the hospital. The discharge planner was glad to be contacted and said they would be sending our agency the orders on discharge.”

Readmission Prevented

“We received a notification of an opportunity to follow up with a patient reporting several clinical concerns, including a recent fall. When I called, she reported she had fallen the day before and had been on the floor for several hours until her son came to visit and was able to help her up. She had hurt her arm, hip, and leg and said she needed HH services. We were able to receive an order that afternoon with the detail we provided from the Cortex notification and my follow-up. Six weeks later in another Cortex follow-up call, this same patient reported that everything was going well, stating she is 'as good as new' now.”
